End of Life/Advance Care Planning

Preparing for the end of life is difficult whether for ourselves or a loved one.  There are community services and supports that can help with planning for the future.  Advanced care planning is thinking about your wishes for the future involving your health and personal care.  Planning and documenting your plan helps for others to respect your wishes should you become ill or unable to communicate these wishes.  Ask your family doctor to speak with our Case manager about how to begin planning or visit.
